Résumé : The previous version of the international classification for lung cancers, based on the TNM and the Mountain series, dated from 1997 and had numerous imperfections. The International Association for the Study of Lung Cancer has undertaken an international large scale retrospective multicentre study (IASLC International Staging Project) which has allowed substantial modifications. The study underlines, for the T descriptor, the role of tumour size, with thresholds at 2, 3, 5 and 7 cm, and of satellite nodules. Malignant effusions are now included in the M1 stage. The new classification is also applicable for small cell lung cancers and carcinoid tumours. A new nodal map is proposed, delineating more precisely the limits of each station. The modifications from the IASLC Staging Project have been accepted by the UICC for the 7th edition of the international classification. © 2009 Société de Pneumologie de Langue Française (SPLF).
