Résumé : Measurement of urinary free cortisol by isotope dilution mass spectrometry. We developed a highly specific method used in routine analysis of urinary free cortisol based on isotope dilution mass spectrometry coupled to gas chromatography (GC-MS). The comparison with our previous immunoassay method (Abbott) showed a significant overestimation of cortisol measurements with the former method though the correlation was good. The lack of specificity led to two false positive out of 106 patients. Cortisol metabolites and synthetic steroids have been identified by GC-MS without interfering in cortisol determination in contrary to immunoassay. Moreover, the great specificity of GC-MS allowed us to reconsider the reference values. © 2001 éditions scientifiques et médicales Elsevier SAS.
