Résumé : Breast reconstruction can be performed safely, giving pleasing results using different techniques. Unfortunately, most of those procedures leave scars on the newly formed breast. This chapter describes a minimally invasive approach to breast reconstruction based on the principles of power-assisted liposuction, loops, and lipofilling (PALLL). This approach avoids visible scarring and enables partial recovery of breast sensitivity. After detaching the subcutaneous adherences from the perimammary tissues, a vascularized adipocutaneous advancement flap is recruited from the upper abdomen and lateral thorax with the use of internal threads (loops). A larger volume of fat can then be grafted in the newly formed breast with minimal skin tension.
