Résumé : Measurement of bone mineral density (BMD) of the central skeleton by dual X-ray absorptiometry allowed tremendous developments in the field of osteoporosis. The World Health Organization defined osteoporosis as a bone density below that of the mean young reference population at the spine, total hip or femoral neck by at least 2.5 standard deviations (SD). Since 50% of the fractures occur in patients who are not classified as osteoporotic, the definition of osteoporosis is evolving to include risk factors other than a low BMD. Models are developed to integrate clinical risk factors in the calculation of absolute fracture risk, the most popular being the FRAX©. New techniques exploring bone structure, another determinant of bone strength, will be briefly described.
