Résumé : Extension of a neoplastic process to the leptomeninges is a relatively rare complication but its incidence is rising in parallel to improved survival of patients with widespread metastatic cancer. It is a severe complication which can, however, be treated effectively. Prophylactic treatment of meningeal recurrence, performed routinely in patients with acute lymphoblastic leukemia, has reduced the incidence of this complication from over 50 to less than 10 per cent. Similar prophylaxis has been considered in other forms of acute leukemia and in non-Hodgkin's lymphomas of the histiocytic type. Eradication of clinically overt meningeal carcinomatosis is always difficult. However, very satisfactory results have been obtained in leukemias with chemotherapy alone. In lymphomas, combined radiotherapy and intrathecal chemotherapy has proven effective. The same treatment is sometimes effective in meningeal carcinomatosis complicating other solid tumors, particularly breast cancer.
