| Résumé : | Objective: Building on a prior study demonstrating increased P300 amplitude sensitivity with a bimodal oddball paradigm in adjustment disorder (AJD) patients, this study compares bimodal P300 characteristics in three psychotic disorder groups—schizophrenia (SZ), bipolar disorder-I (BD-I), and psychotic disorder NOS (PNOS)—against age- and sex-matched healthy controls, aiming to elucidate the bimodal P300′s enhanced sensitivity compared to classical unimodal designs. Method: Three psychotic disorder groups (SZ, BD-I, PNOS) and age- and sex-matched healthy controls (HC-SZ, HC-BD-I, HC-PNOS) performed an oddball task, detecting deviant auditory (A), visual (V), and synchronized bimodal (AV) stimuli by button press. Results: The SZ group exhibited a distinctive lack of typical amplitude variation in the bimodal condition, with prolonged bimodal P3b latencies. The BD-I group showed a more pronounced auditory interference effect on bimodal reaction times and P3b latencies than healthy controls. Notably, bimodal P3b latency measures were key in differentiating the PNOS group from healthy controls, despite absent clear behavioral or unimodal amplitude differences. Conclusions: This study underscores the added value of incorporating a bimodal P300 paradigm alongside unimodal versions, highlighting its enhanced sensitivity in differentiating patient groups with psychotic disorders from healthy controls relative to unimodal paradigms alone. Significance: Consistent with findings in adjustment disorder and major depression, this study suggests psychotic disorders involve altered integrative processes, highlighted by the bimodal P300 paradigm. These electrophysiological distinctions offer a valuable avenue for future research to dissect psychosis’ neurobiological heterogeneity, potentially informing dimensional approaches and personalized psychiatry. |
