Résumé : Patients with acute liver failure (ALF) or acute-on-chronic liver failure (ACLF) had poor short-term prognosis and their severe clinical courses were characterized by accumulation of organ/system failures requiring intensive care unit (ICU) admission and management. The objective definitions of these conditions are now clarified, and prognostic models have been developed. The care is mainly supportive, but some experimental therapies are emerging. Due to this progress, outcomes have been improved. The adequate selection of patients and their clinical optimization for liver transplantation (LT) are the main objectives and challenges. This chapter will summarize the current evidence about the critical care of patients with these clinical entities.
