Résumé : | Thanks to the advances in diagnostic tools and treatments, the number of young cancer survivors is significantly increasing. However, cancer treatments may impair ovarian function and reproductive potential. Oncologists should inform all patients about infertility risks, and offer options to preserve their fertility. This chapter gives an overview of fertility preservation techniques, focusing on the innovating approach of ovarian tissue cryopreservation and transplantation. Further, the two available cryopreservation protocols, slow freezing and more recently vitrification, will be compared. Parameters such as the type of cryoprotectants, follicular survival after thawing, and restoration of the ovarian function posttransplantation are analyzed. Although data on vitrification are encouraging, the slow freezing procedure remains more standardized and the large majority of the pregnancies and live births after transplantation were obtained using this technique. At present, we recommend to use slow freezing as standard and encourage the development of vitrification protocols in the research setting. |
