Résumé : | For about twenty years, a national thesaurus is mandatory available in all belgian primary care electronic health records. This thesaurus of 50.000 dutch and french terms is used for data routine registries, info-button plugin (called evidence linker) and shared summarized health records (called SumEHR). Challenges are to ensure continuity of all the existing data, improve maintenance of this thesaurus and ensure interoperability for continuity of care. The aim was to carry out the legacy conversion of the most frequently used terms to create a Belgian General Pratice Snomed-CT Refset.METHODOLOGY: Terms used more than one time in a sample of six primary care health centers during three years have been extracted. They have been sorted according to the 17 chapters of International Classification of Primary Care (ICPC-2) and reviewed with a blinded dual mapping. Snomed International standards and guidelines have been followed. Every decision has been documented.RESULTS: 8250 of 11099 (74,3 percent) extracted terms have been exactly matched to Snomed-CT concepts. The lowest percentage is 54,2% for chapter Z (social health problems) and the best is 83,2% for chapter N (neurology).DISCUSSION: These results were consistent with international published work. Non-exact matches (narrower, broader or lacking mapping) are still in process. This GP Snomed-CT refset will take place in the collection of national Snomed-CT refsets. International mappings done for ICPC-2, ICD-10 and the general maintenance of Snomed-CT will ease its management. Other works to ensure its usability and quality are to be done. |
