Résumé : In chronic calcifying pancreatitis, obstruction of the main pancreatic duct by a stone may lead to chronic abdominal pain leading to a negative impact on the quality of life for the patient. Subsequently, pancreatic stone removal can play a major role in relieving symptoms. Extracorporeal shock wave lithotripsy is a safe procedure that results in stone fragmentation in more than 90% of patients and has become the first-line therapeutic treatment recommended by the European Society of Gastrointestinal Endoscopy. When the use of extracorporeal shock wave lithotripsy alone provides an unsatisfactory response, it can be combined with endoscopic drainage of the main pancreatic duct resulting in pain relief in two thirds of patients, based on the available data. More advanced techniques such as pancreatoscopy-guided lithotripsy, using an electrohydraulic or laser system, have emerged recently, but further clinical studies are required to determine their place in the management of patients with painful calcifying chronic pancreatitis.
