Résumé : A case of intraparietal hematoma of the basilar artery in a 22 yr old male is described. The authors connect this lesion with an anatomoclinical entity which may be called 'obliterating parietal hemodissection'. It occurs in cerebral vessels (sylvian or basilar artery) free from atherosclerosis, syphilis or medionecrosis. The anomaly of the elastic layer may be overlooked when it is not systematically searched for in all cases of apparent primary thrombosis occurring in the cerebral vessels.
