Résumé : To the Editor: I read with interest the article, “Mineralocorticoid-Induced Hypertension in Patients with Orthostatic Hypotension.”* I certainly agree that it is important to alert physicians to the possible complications of chronic fludrocortisone therapy. For many patients, however, severe orthostatic hypotension is an incapacitating problem, and fludrocortisone should not be withheld. The authors state that recumbent hypertension is an important risk of fludrocortisone therapy. They also report several probable complications of recumbent hypertension, including cardiomegaly and hypertensive retinopathy. In addition, they state that these risks and complications should be considered in any decision regarding long-term fludrocortisone therapy, particularly when the. © 1979, Massachusetts Medical Society.
