Résumé : The criterion which can justify an osteotomy or, on the contrary, a total replacement was studied. Clinical and radiologic results of osteotomies and tenotomies show clearly the reaction of bone to abnormal stresses and define the level of the lesion above which the 'renaissance articulaire' can no more be obtained. This dead end lesion is characterized by the open cyst. It is a clinical criterion of great value which is confirmed by standard and refined radiology, macroscopic and microscopic studies. These have also confirmed that the open cyst, by its internal components, has no power to induce an articular restitutio ad integrum. The open cyst is therefore the criterion which excludes any osteotomy and induces total replacement.
