Résumé : Using microangiography and histological control methods, the authors studied 120 kidneys with vascular nephrosclerosis and pyelonephritis. Two of these cases were found to have renal papillary necrosis (RPN). They describe in detail the vascularization of the normal papilla and demonstrate the absence, in RPN, of primitive arterial lesions. The vascular changes are secondary to a local cicatricial process. The authors conclude that vascular lesions in RPN are non primitive and depend on the combination of several factors: renal disease, hypotension and local accumulation of nephrotoxins. There is also a factor of individual sensitivity which is involved in the origins of papillary necrosis.
