Résumé : A young Zairian patient (25 yr) died from a suspected tuberculous meningitis. This diagnosis was supported by the presence of acid fast bacilli in the sputum and the gastric liquid. At autopsy, performed to discover the primary tuberculous focus, no bacillar lesion was found. Histopathologic examinations disclosed a widespread histoplasmic infestation (Histoplasma Duboisii); there were foci in the brain and heart and massive necrotizing lesions were found in the lymph nodes and testes. There were small foci of infection in the lungs, while the hepatosplenomegaly appeared to be caused by hemosiderosis.
