Résumé : Indications and contraindications for valve replacement in pure or complicated rheumatic mitral insufficiency based on observations in 79 personal cases, the results returned by 13 European Cardiology Centers in reply to a questionnaire, and published data are reviewed. Serious mitral leaks and cardiac decompensation respond badly to treatment and therefore surgery becomes an almost essential indication. From the functional viewpoint, there seems to be a tendency for earlier operations to be carried out (stage IIb); stage IV should not be automatically considered to be a contraindication for surgery. Age, hemodynamic tests, electrocardiograms or radiology merely provide material for discussion. An assessment of myocardial function when this becomes possible would be an important factor and could prevent progressive cardiomyopathy.
