Résumé : It is now quite clear that patients with chronic respiratory failure due to neuromuscular disorders may be helped by night-time ventilation at home, or for some of them by diaphragm pacing. Their quality of life will be markedly improved as soon as the chronic hypercapnia disappears, although the mechanism for this disappearance remains incompletely understood. However, we have to face many ethical as well as practical problems to ensure these patients and their families a safe transition from the hospital to home care, requiring concerted interdisciplinary efforts.
