Résumé : Anemia is common among intensive care unit patients and can have important consequences on myocardial function and, when severe, on the ability to transport oxygen (DO2). Red blood cell transfusion provides a rapid means of correcting anemia, but the potential risks of this strategy (increased blood viscosity, immunomodulation, risks of infection...) must be taken into consideration. There is not one transfusion trigger that can be applied to all patients, and the decision to transfuse must take into account the cause of the anemia, underlying disease processes, and the comorbidities of each patient. In this article, we will focus on patients with cardiovascular disease, cerebral trauma, or septic shock. New randomized controlled studies comparing transfusion triggers in well-defined patient populations are needed to guide transfusion practice. © 2008 Société de réanimation de langue française.
